- Assumption of Risk: I am aware that the activities and services provided by Revive & Thrive Wellness Center LLC, including but not limited to massage, meditation, yoga, reiki, fitness, hypnosis, touch therapy, hypnotherapy, breathing exercises, and other community group events, involve inherent risks and may require physical exertion.
- Physical Condition: I certify that I am in good health and have no medical conditions that would prevent my participation in the activities and services provided by Revive & Thrive Wellness Center LLC.
- Voluntary Participation: I voluntarily choose to participate in the activities and services offered by Revive & Thrive Wellness Center LLC and acknowledge that I am responsible for my own well-being during such participation.
- Release of Liability: I hereby release, discharge, and hold harmless Revive & Thrive Wellness Center LLC, its owners, & service providers/instructors from any and all liability, claims, demands, actions, or causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in the activities or services provided.
- Photography and Video Release: I grant Revive & Thrive Wellness Center LLC. the permission to use photographs and/or videos taken of me during my participation in activities for promotional purposes.
